How many ml is 500 mg?

It depends on what the substance is. There is no fixed conversion, because milligrams measure mass and millilitres measure volume. In water, 500 mg is 0.5 ml. In a syrup at 50 mg/ml, the same 500 mg is 10 ml — twenty times as much liquid.
A milligram is a thousandth of a gram — a mass. A millilitre is a thousandth of a litre — a volume. You cannot convert one into the other any more than you can convert kilograms into metres. What you can do is ask how much mass sits in a given volume, and that number is the concentration (for a solution) or the density (for a pure substance).

Water is the reason people think a fixed factor exists: one millilitre of water weighs one gram, so 1 ml = 1,000 mg. That is a property of water, not a rule of arithmetic.

Find the strength on the label
A bottle says 250 mg / 5 ml. That is the strength, not yet the concentration.
Divide it down to one millilitre
250 ÷ 5 = 50 mg/ml. This is the number the calculator wants.
Convert your amount
For 500 mg: 500 ÷ 50 = 10 ml.
Sanity-check the direction
More liquid than milligrams means the concentration is below 1,000. If your answer moves the wrong way, the direction select is probably flipped.
The number is only as good as the concentration you entered, so it is worth being sure where that came from.
For a solution, it is on the packaging — as mg/ml, as a strength like "250 mg/5 ml", or as a percentage. A 1 % solution is 10 mg/ml, a 0.9 % saline is 9 mg/ml.
For a pure liquid, it is the density. Water 1,000 mg/ml, ethanol about 789, cooking oil around 920, honey about 1,420.

Do not use this to calculate a medication dose
This is a unit converter. It does not know what your medicine is, what strength you actually have, or what dose is right for anyone.
Medication strengths vary between formulations of the same drug, paediatric and adult preparations differ, and a misplaced decimal in a concentration produces an error of ten or a hundred times. Read the label, follow the prescribed dose, and ask a pharmacist or doctor if anything is unclear. Use the dosing device that came with the medicine rather than converting to a household spoon.
Two further limits: the arithmetic assumes the concentration is uniform, so shake suspensions before measuring; and densities shift slightly with temperature, which matters for precision laboratory work but not for everyday conversions.
